Mental Health Intensive Outpatient is an independent educational resource, not a healthcare provider — but we hold anything you send us to the standard the Health Insurance Portability and Accountability Act (HIPAA) sets for protected health information (PHI). What you share with us is confidential, and we safeguard it and will notify you if a breach affects it.

Your Rights Under HIPAA
Access and Copies
At any healthcare provider, you may inspect and request a copy of your medical record, in most cases within thirty days of a written request.
Amendment
If you believe something in your record is inaccurate or incomplete, you may ask the provider in writing to correct it.
Restrictions and Confidential Communication
You may request limits on how your information is used or disclosed, and ask a provider to contact you only at a specific number or address.
Substance Use Records and Additional Protections
Records relating to substance use treatment carry protections beyond HIPAA under federal regulation 42 CFR Part 2. In general, these records cannot be disclosed without your specific written consent, including to family members, employers or law enforcement, except in narrowly defined circumstances such as a medical emergency or a court order meeting the regulation's requirements.
There are limited situations in which the law requires disclosure regardless of consent, including suspected abuse or neglect of a child or vulnerable adult, and circumstances involving imminent danger to yourself or another person. Providers are expected to tell you when they are obligated to make such a disclosure whenever it is safe and practical to do so.
